New riders UNDER 18 years old

• If you are a new rider to DDASC, only parents or legal guardians are authorized to fill out and sign your waiver. This means that other relatives or friends, regardless of age, will not be able to authorize you to ride at DDASC unless they have a certified document showing proof of guardianship. 
•  You cannot fill out and sign your own waiver.​
• If your parents or legal guardians cannot be with you when you arrive to DDASC to fill out your waiver, they will be able to fill it out through our e-form.​
• Authorizations via email, telephone, or text message will not be valid.
• Keep in mind that if you don't have a valid waiver, you won't be allowed to ride at our sports facility.

• If you are a new rider to DDASC, you can fill out and sign your own waiver at our front desk. You can also download the PDF, fill out the information, sign it at home, and bring it ready with you.

• You can also fill out the waiver through our e-form.

• Please enter a valid emergency contact such as friends and relatives, regardless of if they are in town with you or not. 911 is not a valid emergency phone number.

• Keep in mind that if you don't have a valid waiver, you won't be allowed to ride at our sports facility.
